Part I — Leave Request

  1. Control number
  2. Name: Last name, first name, and middle initial

  7. Type of leave: Ordinary, emergency, permissive TDY, or other
  8. Organization, station, and telephone number

  12. Supervisor recommendation and signature: Approval or disapproval
  13. Signature and title of approving authority
  14. Departure: Date, time, and the name, title, and signature of the departure authority
  15. Extension: Number of days, approval date, and the name, title, and signature of the approval authority
  16. Return: Date, time, and the name, title, and signature of the return authority
  17. Remarks: Includes the period of chargeable leave

Part II — Emergency Leave Transportation and Travel

The traveler is authorized to proceed on official travel in connection with emergency leave and, after completing leave and travel, return to the home station or other location designated by military orders.

The traveler must report to the Aerial Port of Embarkation for onward movement to the authorized international airport identified in the travel documents. Additional travel is chargeable to leave.

Do not depart the installation without reservations or tickets for authorized space-required transportation. File a no-pay travel voucher with copies of the travel documents or boarding pass within five working days after returning. Requests for leave extensions must be submitted to the commander. The American Red Cross may assist with notifying the commander about an extension request.

Part III — Dependent Travel Authorization

Records whether authorized dependent transportation is one way or round trip and whether it is space available, space required, or cash reimbursable.

  • Date of birth for children
  • Passport number

Part IV — Authentication for Travel Authorization

  1. Designation and location of headquarters
  2. Accounting citation
  3. Date issued
  4. Travel order number
  5. Order-authorizing official: Title and signature or authentication
